WHAT IS A CARE PLAN

A care plan is simply the plan that outlines how a person’s care needs will be met. It ensures that the individual receives the right support and that everyone involved in providing care understands exactly what is needed and expected.

In some cases, such as when you’re working with a home care agency, health provider, or care management organization-the care plan is a formal document created and shared with you. It typically includes goals, specific services, schedules, and responsibilities.

If you’re managing care independently, your “care plan” might be less formal-perhaps a folder or notebook containing schedules, medication lists, appointments, and daily care tasks. This still serves the same purpose: organizing and guiding the care process.

Our staff of professional Health Care Advocates are ready to step in to help support your loved one’s health and independence. When you enlist EverHome Care Advisors’ services, you’ll be assigned a Health Care Advocate who will interview you to determine your goals and needs.  They may conduct in-home assessments to identify needs and make individualized recommendations.  They can also take care of the ongoing care coordination needed to make sure care is maintained and re-evaluated as needed.  Our staff can do the time-consuming research and ‘’leg work” needed to access the resources and get you to a plan that works for both all involved.
