The EverHome Care Advisors staff have the knowledge, skills, and expertise to guide you through the complex care delivery systems. As Aging Life Care Professionals we know how to navigate that system. Whether it is home care, acute care, acute rehabilitation/sub-acute care or long-term care, we know what your loved one’s rights are and can assist you through the care and/or insurance appeal process. Your Aging Life Care Coordinator can intervene when:

• You think you or your loved one is being discharged from a hospital or rehabilitation facility too soon
• Your insurance plan is denying care or reducing hours of needed care
• You are not happy with the level of care provided in a facility
• Your loved one needs placement in an alternative care facility such as an assisted living or skilled nursing home

Our staff supports clients and families by guiding them through the care process and helping them understand the normal routine processes and procedures that occur in a care setting and their rights as a patient in a health care setting. Letting us be your advocate helps you make more informed care decisions for your loved one, manage your time more efficiently, and relieves the stress of making important decisions under pressure.

Being proactive with getting a comprehensive assessment and developing a plan of care to manage your loved one is key as a caregiver.
