Sherrie Catapano is guided by a deep commitment to integrity, compassion, and respect for the dignity of every individual. With a foundation in social work, she believes in the power of human connection and the importance of honoring each person’s and family’s unique perspective. Sherrie champions client-centered care and self-determination, using her exceptional listening skills to build trust, understand personal stories, and help individuals find solutions that ultimately would align with their goals and needs.

Experience
Sherrie has over 30 years of experience as a social work case manager in the healthcare industry. In 2023 she retired from a local twenty-six years of service.  Sherrie has supported countless individuals, caregivers, and families through some of life’s most challenging transitions. Her role involved navigating the complexities of the healthcare system, ensuring that care coordination was seamless and that every voice was respected and heard.

Recognizing the environmental and personal factors that shape an individual’s daily life is the foundation of Sherrie’s clinical practice. 

She has partnered extensively with multidisciplinary providers and community-based organizations, working together to promote individual health and wellness. Through these partnerships, she has helped drive meaningful improvements in care delivery, always prioritizing the needs and dignity of the client.

Background
Sherrie’s journey into social work began with volunteer work at a Schenectady County Community Action program, where she witnessed firsthand the challenges older adults face in accessing care. After raising her family, Sherrie returned to college to obtain her Master’s degree to become a NYS Licensed Master Social Worker and a Certified Case Manager. 

Her internship was at Community Health Plan (CHP), a health maintenance organization (HMO) in Latham NY where she gained invaluable knowledge and experience about the health care field.  Her journey in the healthcare field began with a deep commitment to client-centered care and a passion for improving the quality of health outcomes.  

 Her work has consistently demonstrated the transformative impact of healthcare advocacy—especially for older adults navigating complex medical and social needs. Sherrie believes that case management is essential to improving quality of life and achieving better health outcomes.

Leadership & Professional Involvement
To further her professional development, Sherrie joined The Case Management Society of America (CMSA) in 2011.  CMSA is the leading, oldest, and largest membership association providing professional collaboration and education across the health care continuum. Since 2018 Sherrie has proudly served as the President of New York Capital Region Case Management Society Chapter.  

Personal Interests
Beyond her professional life, Sherrie finds joy in spending time with her family and exploring the world through travel. She embraces life with curiosity and a sense of adventure, always seeking new experiences and meaning
